Abstract Multi-organ fibrosis among end stage renal disease (ESRD) patients cannot be explained by uremia alone. Despite mitigation of thrombosis during hemodialysis (HD), subsequent platelet dysfunction and tissue dysregulation are less understood. We comprehensively profiled plasma platelets from ESRD before after HD to examine HD-modulation beyond thrombotic activation. Basal levels proteolytic regulators fibrotic factors were elevated in compared healthy controls, with isoform-specific changes HD. Platelet lysate (PL) RNA transcripts for growth coagulative post-HD, upregulation correlated vintage. secretome correlations reveal acutely induced pro-fibrotic phenotypes characterized preferentially enhanced enzyme translation secretion, contribution inflammatory response, increasing blood flow rate (BFR) Vintage. Compensatory mechanisms increased factor synthesis acute matrix metalloproteinase (MMP) inhibitor MMPs (TIMP) increases show short-term mode-switching between dialysis sessions leading long-term bias. Chronic adaptation observed through differential secretory kinetics heterogenous granule subtypes. conclude that chronic responses contribute a milieu ESRD.
